B1UP issue - In the Form-Settings Manager I only have one Form in the list to copy.

Open Tools > User-Defined Windows >  > BOY_81FORMTYPES - B1UP Manager: Form Types.

If there is only one record in there try to delete it and restart the add-on. Then all options should be shown
