Try editing the Form's IPT configuration for the UDF. 1) Set the Type field to "ComboBox" 2) Set  the Valid Values field to a SQL statement "SQL:SELECT Code, Name FROM [@MY_TABLE] WHERE U_ACTIVE = ...
