MailChimp Report request

Could you please assist us with a report for Mail Chimp

Our client would like to get a list with all customers who subscribed to one of our MailChimp Lists, indicating to which specific list they subscribed.
The OCPR table only shows status unsubscribed, and the other reports do not have this information.
